This clinic will be held at Bentonville Fire Station #1 located at 800 SW A Street, from 8:00 a.m. to 5:00 p.m. Vaccine shots will be administered by the Bentonville Fire Department.
This clinic is for people that are included in phase 1-A and those that are eligible for 1-B.
The only groups eligible at this moment, are persons residing in Arkansas who are age 65 or older, teachers or school staff, child care workers, higher education workers, food manufacturing workers including meat processing and oilseed milling, and Phase 1-A eligible.

You will be temperature checked upon entry, then asked to complete a questionnaire, sign a waiver, and provide some additional information, including your red, white and blue Medicare card, if you have it. If a Veteran has served in the military but is not currently enrolled, the VHSO eligibility team will be happy to assist them to see if they qualify to be enrolled. Individuals will receive an automatic appointment to get the second dose of the Moderna vaccine.
