UAMS Northwest is hosting weekday drive-thru vaccine clinics by appointment only. To get your vaccine, you must live in Arkansas and will need to provide:

Photo ID
Phone Number
Date of Birth
Name

You will NOT need to provide a social security number, proof of citizenship or legal status, or insurance.

Register at vaccinesignup.uams.edu/nwaclinic or by calling 479-332-0222.

If you call and do not receive an answer, please leave a voicemail and you will be contacted to schedule your appointment.
